I took EM comat my 4th year. I spent the entire month watching kdramas and the night before the comat I took a red eye across the country, landed 2 hrs before the comat, got to where I was staying and fell asleep during the comat for a little bit. There's like 450 questions for FM and like 1000+ for "Medicine". I felt like it had a good spread of systems like you mentioned but it tended to be more outpatient management oriented. FM comat is easier. Lots of guideline based questions like what age to do colonoscopy and stuff. FM was a bit easier and it was more outpatient based.

Used COMQUEST for everything except the OMM shelf and surgery (surgery was my first rotation and I didn't know I needed a qbank and I hate OMM), and those two were the only scores below 100 for me.
